>There are always recall notices on bicycle components. Check to make sure your fork wasn't recalled. As a rule I stick to carbon forks with a life time warranty. I avoid anything that has a one year warranty or is a no name brand import.

What brand fork was it?!?

Did the fork have a carbon steerer or an aluminum steerer?

Was the headset properly installed?

Did you OR a shop do the installation?

Undoubtedly, there may be liability issues which would be either mitigated or aggravated by the level of YOUR involvement in the installation and maintenance.
