It is measured from the top, 'flat part', of the handlebar to the lowest flat section, right before the bar ends. "Shallow" and "deep" are relative terms, shallow meaning the ends of the bar aren't that far down below the top, deep meaning the bottom of the bars are a ways down. Whether you get a deep or shallow drop bar depends on your size, the use of the bike, and personal preference. Shallow drop bars, from what I know, are the typical bars found on most road bikes that are around, deep drop bars are used for large bikes, for tall riders, and track bikes. Deep drop bars allow you to get lower and in a tighter tuck than shallow drop bars, if both bars are set to the same height at the stem. The curve in deep drop bars is also larger (compared to shallow) which is more comfortable for larger hands.
